Print and paperless: Back-to-school student information form to learn more about your secondary students at the beginning of the school year. This resource comes in print and online file options. A fully editable version is also included (Editable Power Point file), so you can adapt this resource to better suit your needs. Please note that the design is not editable, only the texts. The online file is not editable; it only contains editable boxes that students may fill in with the required information.

This resource is more appropriate for grade 7-12 ESL/EFL/ELA learners, but it may also be suitable for other subjects or students below the indicated grade levels.
